## Description

We are seeking a Band 5 Nurse to work at Cobham and Oxshott PCN, within Surrey Downs Health & Care. As a Community Nurse, you will be responsible for assessing care needs and developing, implementing, and evaluating holistic care packages. You will work as part of a team, contributing to team management and the development of innovative practice.

Responsibilities:

- Assess individual care needs and implement holistic programmes of care

- Involve, support, inform, and educate families and carers

- Promote health and wellbeing of patients

- Develop effective communication and liaison systems

- Maintain clear and comprehensive records

- Facilitate training and professional updating for teams and students

- Act as an assessor and mentor to qualified and unqualified staff

- Maintain standards of evidence-based nursing care

- Support and mentor team members and participate in clinical supervision activities

- Work without direct supervision and maintain close liaison with senior team members

- Be responsible for timely patient care and data entry

- Work in co-operation with other professionals and agencies involved in patient care

Requirements:

- Registered Adult Nurse with NMC

- Valid Driving Licence (to drive within UK)

- Ability to assess, deliver, and evaluate quality care

- Effective communicator

Salary: £33,677 to £40,996 a year Pro Rata Per Annum inc Fringe HCAS
