## Description

We are seeking a Physical Therapist (PT) for a float position in the Good Samaritan and Dreyer areas. As a Physical Therapist with Advocate Health, you will play a crucial role in interpreting physician referrals, completing evaluations, and establishing plans of care for patients. You will work with a team to provide high-quality patient care and promote optimal outcomes.

Responsibilities:

- Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations

- Establish a plan of care and implement individual treatment based on evaluation results and functional goals

- Monitor and evaluate outcomes, adjusting the treatment plan as needed

- Provide patient/family teaching, referral for community services, and transition planning

- Complete required documentation, including patient evaluations, progress notes, and discharge summaries

- Participate in performance improvement activities and pursue professional growth through continuing education

Requirements:

- Doctorate degree in Physical Therapy from an accredited school

- Licensed Physical Therapist in the state where you work

- Basic Life Support (BLS) for Healthcare Providers certification

- Intermediate computer skills and excellent communication, interpersonal, and organization skills

Benefits:

- $5000 sign-on bonus payable in two installments

- Float differential $5.00 per hour and weekend differential $2.00 per hour
