## Description

We are seeking a Physical Therapist (PT) - Home Health Case Manager to join our team at Advocate Health. As a Physical Therapist, you will play a crucial role in ensuring patients receive quality, cost-effective care in the comfort of their own homes.

**Responsibilities:**

- Assess patients' physical needs and develop personalized plans of care

- Collaborate with patients, families, and healthcare teams to achieve optimal health outcomes

- Utilize evidence-based practices and stay up-to-date with the latest standards in physical therapy

- Participate in quality improvement initiatives and promote patient safety

- Communicate effectively with patients, families, and healthcare professionals

- Prioritize and organize patient care, ensuring clear hand-off communication

- Initiate referrals and discharge planning for assigned patients

**Requirements:**

- Physical Therapy License in the State of Illinois

- Basic Life Support (BLS) for Healthcare Providers certification

- Valid driver's license

- Doctorate degree in Physical Therapy from an accredited school

- 1 year of clinical experience or strong spectrum of clinical affiliation experience

- Basic knowledge of computer use and ability to learn computer-based scheduling and documentation systems

- Strong communication and interpersonal skills

**Benefits:**

- Full-time position, 40 hours per week

- Monday - Friday, 8am-5pm, with rotation to weekends and 1 summer/winter holiday
