## Description

We are seeking an experienced Clinical Specialist Nurse to join our Gastroenterology team at Wexham Park Hospital. As a Clinical Specialist Nurse, you will play a key role in delivering high-quality, patient-centred care for people living with Inflammatory Bowel Disease (IBD).

You will work autonomously as a specialist practitioner, providing expert assessment, advice, education, and support throughout the patient journey. You will collaborate with multidisciplinary teams to ensure patients receive timely, safe, and effective treatment.

Key responsibilities include:

- Providing specialist assessment, treatment, education, and support for patients with IBD

- Managing nurse-led telephone and email advice services

- Coordinating outpatient care and patient follow-up

- Monitoring patients receiving biologic and advanced therapies

- Identifying and managing disease flare-ups promptly

- Promoting patient self-management and education

- Supporting discharge planning and care coordination

- Working collaboratively with consultants, pharmacists, dietitians, and primary care teams

The role also involves contributing to service development, quality improvement, clinical audit, education, and implementing evidence-based practice and national IBD standards.

The salary for this position is £41,957 to £50,387 per annum, inclusive of HCAS.
