## Description

We are seeking a passionate and organised Connected Care Lead to join our team at New Victoria Hospital. As Connected Care Lead, you will become a central point of coordination for patients accessing specialist care pathways, ensuring their journey is efficient, supported and of the highest quality.

You will:

- Coordinate referrals, bookings and patient pathways across multiple Bupa services.

- Act as a key liaison between patients, consultants, clinical teams, diagnostics services and Bupa Insurance.

- Monitor patient progression and proactively resolve any pathway delays.

- Coordinate and administer up to three specialist Multi-Disciplinary Teams (MDTs).

- Organise MDT meetings, prepare documentation and ensure outcomes are accurately recorded and communicated.

- Support pathway performance monitoring and service improvement initiatives.

- Build strong relationships with internal and external stakeholders.

- Contribute to the continued development and growth of Connected Care services.

- Knowledge of CQC and healthcare regulatory standards
- Experience of healthcare performance metrics, SLAs and service improvement methodologies
- Degree level qualification
