## Description

We are seeking a Physical Therapist to join our innovative float team at Aurora. As a Physical Therapist, you will interpret physician referrals, complete musculoskeletal evaluations, establish plans of care, monitor patient outcomes, and adjust treatment plans as needed. You will also participate in performance improvement activities, pursue professional growth, and orient new staff to clinical and department policies.

Responsibilities:

- Interpret physician referrals and complete musculoskeletal evaluations

- Establish plans of care and implement individual treatment based on evaluation results and functional goals

- Monitor and evaluate patient outcomes and adjust treatment plans as needed

- Plan for and complete patient discharge, including providing written home programs and patient/family teaching

- Complete required documentation, including patient progress notes and discharge summaries

- Clean up after patient treatment and maintain a neat and orderly workspace

- Demonstrate skill in use of modalities, exercise equipment, and other devices

- Participate in performance improvement activities and pursue professional growth

Requirements:

- Physical Therapist license issued by the state in which the team member practices (Wisconsin or Illinois)

- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA)

- Bachelor's Degree in Physical Therapy

- Intermediate computer skills

- Excellent communication and interpersonal skills

- Demonstrated organization, decision-making, and problem-solving skills
