## Description

We are seeking a Senior Discharge Facilitator (Advanced Social Worker) to join our hospital discharge/LIFE team. The successful candidate will be responsible for case managing and facilitating safe and effective discharges of patients from hospital to a community setting through a multidisciplinary approach.

Key responsibilities include:

- Liaising with the hospital and professionals to facilitate timely discharges

- Undertaking Strength/Asset Based/Assessments and or reviews, sign, post, respond to enquiries and to implement support packages where required

- Completing Care Act Assessments to determine ongoing care needs

- Providing information and advice

- Working in partnership with health professionals in the Integrated Discharge Team

- Triaging safeguarding initial concerns raised, information gathering and participating in SAM discussions

- Working in partnership with service users, carers and family members

- Ensuring a high standard of case recording

- Completing risk assessments and implementing protection plans as required

- Undertaking Mental Capacity Assessments when required

- Completing referrals to Advocacy services

- Deputising in the manager's absence
