## Description

We are seeking a Physical Therapist to join our team at Advocate Health Care in Waukesha, WI.

As a Physical Therapist, you will interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations. You will establish a plan of care, implement individual treatment, monitor and evaluate outcomes, and adjust the treatment plan as needed.

Responsibilities:

- Interpret physician referrals and complete evaluations

- Establish a plan of care and implement individual treatment

- Monitor and evaluate outcomes and adjust the treatment plan

- Provide status updates to physicians and other teammates

- Identify factors affecting patient's physical and psychosocial comfort

- Plan for and complete patient discharge

- Communicate and collaborate with stakeholders

- Complete required documentation and maintain productivity standards

- Clean up after patient treatment and maintain a safe working environment

Requirements:

- Doctorate degree in Physical Therapy from an accredited school

- Licensed Physical Therapist in the state

- Basic Life Support (BLS) certification

- Intermediate computer skills

- Excellent communication, interpersonal, and organization skills

Preferred Requirements:

- Pelvic health and ortho experience

- Experience or interest in learning male and female pelvic health

Benefits:

- Sign-on bonus: $5,000 for full-time, $2,500 for part-time
