## Description

We are seeking a full-time Physical Therapist to join our Outpatient Rehabilitation team at Advocate Health's Aurora Medical Center Oshkosh: Fond du Lac Campus.

As a Physical Therapist, you will interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations. You will establish a plan of care, implement individual treatment, monitor and evaluate outcomes, and adjust the treatment plan as needed.

Responsibilities:

- Evaluate patients and develop plans of care

- Implement individual treatment plans and monitor patient progress

- Collaborate with physicians and other healthcare professionals

- Complete required documentation and maintain accurate records

- Participate in performance improvement activities and contribute to quality of care

- Supervise physical therapy assistants and students as needed

Requirements:

- Doctorate degree in Physical Therapy from an accredited school

- Licensed Physical Therapist in the state where you work

- Basic Life Support (BLS) certification

- Intermediate computer skills and excellent communication, interpersonal, and organization skills
