## Description

We are seeking a full-time Home Care Physical Therapist to join our team in Lincoln. As a Home Care Physical Therapist, you will be responsible for managing a caseload of patients, ensuring timely care and documentation, and collaborating with a multidisciplinary team to provide high-quality patient outcomes.

**Responsibilities:**

- Manage daily patient caseload, ensuring timely care and documentation

- Collect and analyze data to determine patient needs and resources

- Prioritize patient care based on criticality of health and welfare

- Evaluate patients to determine rehabilitation needs and develop plans of care

- Implement plans of care, perform physical therapy services, and direct care coordination

- Maintain communication with multidisciplinary team and physician oversight

- Incorporate short and long-term goals into therapy and document progress

**Requirements:**

- Doctor of Physical Therapy (DPT) degree required

- Physical Therapy License in the state of practice required

- Basic Life Support for Healthcare Provider certification required

- Current state driver's license and auto insurance required

- Personal vehicle must be well-maintained and in safe driving condition

**Preferred Requirements:**

- 1-year experience as a licensed PT in a clinical care setting or home health preferred

- Pediatric experience within the past 5 years preferred

**Benefits:**

- Competitive salary range: $41.10 - $61.65
