## Description

Evernorth Workplace Care offers health care delivery services along with population health and health coaching solutions, conducted in person or virtually. The mission is to deliver proactive, personalized, and holistic patient care and coaching by acting on health data and insights to improve the overall health and wellness of clients' employees, and those they care about most.

As a RN - Care Coordinator, you will:

- Identify and refer customers to eligible programs within CIGNA Healthcare and client-specific health and wellness programs

- Support care coordination of high-risk targeted populations in an efficient and cost-effective manner

- Partner with employers, providers, and Evernorth programs to support improved outcomes

- Analyze patient-level data to identify customers for outreach and engagement

- Maintain timely and compliant documentation and information exchange

- Communicate clearly with matrix partners and refer to internal and external programs

- Identify cases suitable for complex case management and follow referral processes

- Work effectively with Evernorth's internal team to support care coordination and data analysis

- Build relationships with local network providers and provide steerage to high-quality network providers

- Identify gaps in care and work with providers to close them

- Establish and maintain positive, professional relationships with customers, employers, and providers

Requirements:

- RN with current unrestricted state licensure in Ohio

- Three or more years of clinical practice experience

- Case management experience preferred

- Excellent communication and computer skills

- Ability to work in a team environment and exhibit flexibility and enthusiasm

- Current BLS/CPR/AED certification

- Strong problem-solving skills

Preferred qualifications:

- Multistate licensure

- CHES or NBHWC

- Health coaching experience

- Experience working with high-risk patients

- Project development and matrix environment experience

This position is based in Cleveland, OH, and offers an annual salary of $77,500 - $129,100, depending on experience and location. The role is also eligible to participate in an annual bonus plan.
