## Description

We are seeking a qualified Social Worker or Senior Social Worker to join our Community Adult Social Work Team at the Civic Centre in Uxbridge. As a Social Worker, you will support residents with a range of needs and help them improve their well-being.

Key responsibilities include:

- Conducting Care Act assessments and developing individual support plans

- Completing reviews for packages of care already in place

- Participating in team meetings and professional forums to share knowledge and expertise

- Applying a preventative and strength-based approach in your practice

- Conducting S42 investigations to safeguard vulnerable adults from abuse or neglect

As a Senior Social Worker, you will manage a caseload and may have supervisory responsibility for up to two workers within the team.

We are looking for Social Workers who possess excellent communication skills, the ability to apply research-informed practice in interventions, and a collaborative approach.
