## Description

We are seeking a Salaried General Practitioner to join our team at Valentine Primary Care Network (PCN) in Greenwich. As a General Practitioner, you will work autonomously and with other healthcare professionals to assess, diagnose, and treat patients in a primary care setting.

**Job Summary** The post holder will be employed by Greenwich Health, the GP Federation in Greenwich. Greenwich has 7 primary care networks (PCNs) with list sizes of between 30-80,000 patients. The Post holder will work for Valentine PCN.

**Main Duties of the Job**

- Conduct surgeries and online consultations, telephone consultations, and queries

- Visit patients at home and check and sign authorise repeat prescriptions

- Deal with queries, paperwork, and correspondence in a timely fashion

- Screen patients for disease risk factors and early signs of illness

- Develop care plans for patients in consultation with them and in line with current practice disease management protocols

- Provide counselling and health education

- Admit or discharge patients to and from the caseload and refer to other care providers as appropriate

- Record clear and contemporaneous consultation notes

- Compile and issue computer-generated acute and repeat prescriptions

- Prescribe in accordance with the practice prescribing formulary

**Responsibilities**

- Provide expert professional advice to patients, carers, and colleagues

- Ensure the maintenance of clinical excellence and be professionally accountable and responsible for all elements of clinical practice

- Participate in the provision of education and training to GP Registrars and other staff

- Work in PCN-wide schemes/services, including some extended hours and potential hub working
