## Description

We are seeking a Physical Therapist (PT) to join our Home Health team in the South Sub West Suburbs. As a Physical Therapist, you will play a crucial role in providing high-quality patient care in a home health setting.

**Responsibilities:**

- Assess patients' physical needs and develop plans to help them recover from injuries or illnesses

- Provide therapy sessions to patients in their homes, focusing on improving mobility, strength, and function

- Collaborate with other healthcare professionals to ensure comprehensive care

- Educate patients and families on exercises, treatments, and self-care techniques

- Document patient progress and adjust treatment plans as needed

**Requirements:**

- Licensed Physical Therapist in the State of Illinois (or eligible for licensure)

- 1 year of clinical experience or strong spectrum of clinical affiliation experience

- Basic knowledge of computer use (Microsoft Word, email, etc.)

- Ability to learn computer-based scheduling and documentation systems

- Excellent communication and interpersonal skills

- Strong time management and self-organization skills

- Proficient use of home-based therapy equipment and assistive devices
