## Description

We are seeking a full-time Physical Therapist to join our Outpatient Rehabilitation team in Sheboygan, WI.

The Physical Therapist will interpret physician referrals, complete evaluations, establish plans of care, monitor patient outcomes, and adjust treatment plans as needed.

Responsibilities:

- Interpret physician referrals and complete musculoskeletal, neuromuscular, cardiopulmonary, and integumentary evaluations

- Establish a plan of care and implement individual treatment based on evaluation results and functional goals

- Monitor and evaluate outcomes, adjusting the individualized treatment plan based on patient response to treatment

- Plan for and complete patient discharge, including providing written home programs, patient/family teaching, and referral for community services

- Complete required documentation, including patient progress notes, team conference reports, and discharge summaries

- Clean up after patient treatment, put away equipment, and maintain a neat and orderly work area

- Demonstrate skill in use of modalities, exercise equipment, and other devices

- Participate in performance improvement activities and manage assigned caseload

- Pursue professional growth through continuing education courses and department inservices

Requirements:

- Physical Therapist license issued by the state of Wisconsin

- Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA)

- Bachelor's Degree in Physical Therapy

- Intermediate computer skills, excellent communication and interpersonal skills, and demonstrated organization, decision-making, and problem-solving skills
