Coordinates all case management activities with members to evaluate medical needs and facilitate overall wellness. Develops strategies to address issues and enhance member wellness through integration.

**Responsibilities**

- Instructs programs and procedures in compliance with network management and clinical coverage policies.

- Coordinates case management activities with members.

- Develops strategies to address issues and enhance member wellness.

**Requirements**

- Current/Active Clinical Licensure (RN)

- Case Management experience

- Strong customer service orientation and problem-solving skills

- Excellent communication and telephonic skills

- Motivational interviewing skills and ability to build rapport and trust telephonically

- Excellence in documentation and compliance with regulatory standards

- Highly organized and self-driven without direct supervision

- Ability to manage time effectively and achieve performance metrics

- Technology proficiency

- Ability to travel within 125 miles round trip for 1 day in the field

- Variable work schedule with ability to work 2 days weekly until 9 pm local time.

**Preferred Qualifications**

- Bachelor's or Master's Degree

- CCM Certification

- Previous experience in a Managed Care Organization
