## Description

We are seeking a full-time Physical Therapist to join our team at Wake Forest Baptist Medical Center - Outpatient Rehabilitation: Physical Therapy in Winston Salem, NC.

As a Physical Therapist, you will be responsible for providing high-quality patient care, evaluating patients, developing treatment plans, and monitoring progress.

**Responsibilities:**

- Provide age-appropriate patient care, demonstrating knowledge of growth and development principles across the lifespan

- Interpret medical records and provider referrals, completing evaluations and establishing patient care plans

- Develop and implement treatment plans, including education, exercise, and modalities

- Monitor patient progress, adjusting treatment plans as needed

- Communicate with patients, caregivers, and healthcare professionals to ensure coordinated care

- Maintain accurate and timely documentation of patient care and treatment

- Participate in performance improvement initiatives and professional development activities

**Requirements:**

- Physical Therapy License in the state of employment

- Basic Life Support (BLS) certification for Healthcare Providers

- Degree in Physical Therapy from an accredited institution

- Doctor of Physical Therapy (DPT) preferred

- Continuing education requirements as mandated by applicable state professional standards

**Schedule:**

- Monday - Friday, 8am - 5pm
