## Description

Priory Hospital Suttons Manor, located in Stapleford Tawney, Romford, has an opportunity for a Healthcare Support Worker to join their team. The hospital offers a wide range of support to people with complex mental health needs, working in a forensic low secure service for male patients with mental health needs and physical health challenges.

Key responsibilities include:

- Assisting qualified staff with the assessment and implementation of individual patient care plans

- Undertaking routine tasks and activities as directed or delegated to facilitate wellbeing, dignity and treatment of patients

- Ensuring that patients have their personal needs or requirements met, including personal care, participation in activities and support with daily living tasks

- Completing daily administration, including updating person-centred support and care plans, updating charts and completing accurate daily notes

- Maintaining professional relationships with patients, families, colleagues, visitors and other stakeholders

- Supporting patients in achieving their outcomes, and ensuring that these are comprehensively documented and reviewed
