## Description

We are seeking a skilled, compassionate and enthusiastic Practice Nurse to join Modality East Surrey Medical Practice. Caring for approximately 45,000-46,000 patients across four sites, we offer the opportunity to deliver the breadth of practice nursing within an innovative, forward-thinking training practice.

The Practice Nurse will deliver safe, evidence-based nursing care across the East Surrey practice, working within NMC professional standards, demonstrated competence and agreed clinical protocols.

Core responsibilities include:

- Nurse-led reviews and support for long-term conditions, including diabetes, asthma and COPD, according to training and competence.

- Cervical screening, immunisations and vaccinations with appropriate training and authorisation.

- Wound assessment, dressings, suture removal and other routine nursing procedures within competence.

- Preventative care, screening and health checks, with personalised lifestyle and self-management advice.

- Recognition and escalation of deterioration and safeguarding concerns.

- Accurate clinical documentation, infection prevention and safe handling of medicines and equipment.

- Collaboration with specialist nurses, GP partners and the wider multidisciplinary team.

- Participation in audit, quality improvement, continuing professional development and sharing learning.
